Patel distinguishes the presented posterior mandibular D2–D3 case from two other anterior D1 cases. Patel also explains the difference between displacing the buccal plate during split crest and expanding the bone. The video about pterygoid implants is a separate contribution in the same discussion.
To do ridge split in D1 is very challenging the case presented was posterior mandible D2-D3 bone
But still its possible to do ridge split in D1 - anterior mandible i have done 2 cases , but we need to give vertical cuts in bone anterior and posterior to ridge split site and some time also horizontal cut in cortical bone at base of osteotomy
Here we are not expanding bone but we want to push buccal plate outside so i force setting of mallet is not much important I generally use setting 2 only
Force setting is more repevant when we are doing expansion
And i never do expansion in D1 and D2 bone i think not recommended also becuase chance of bone necrosis is there
I also never use magnetic mallet in D1 , D2 bone except ridge split becuase in ridge split forces are not concentrated in bone and dont cause pressure necrosis they get relieved due to bone split
