7–8 mm of residual bone: split crest, osteotomes or a short implant? What about the nasal floor?

Ridge expansion and split crestDecide how to proceed

Good morning, everyone. How would you approach this ridge? A split-crest procedure? Osteotomes alone, with a possible buccal membrane?

Another question: can we perform small nasal-floor elevations without difficulty, as we would in the maxillary sinus?

Altra domanda… nel pavimento nasale possiamo fare mini rialzi senza problemi giusto? Come se fosse il seno mascellare…

Hello. From my perspective: - There appears to be sufficient bone to place an implant without horizontal or vertical augmentation. The prosthetic requirements should perhaps guide the decision. Could soft-tissue augmentation alone be sufficient? - Regarding the nasal floor, several years ago I attended a cadaver course with Chiapasco, who strongly advised against this approach because of the risk of altering the patient’s airway. In some cases, I perforate the cortical plate and place the implant apex slightly beyond the nasal-floor cortex, but I have never inserted biomaterials there. How much can the floor actually be elevated—2 mm? At that point, it may be preferable to place a shorter implant, since these have now been shown to perform well.

Place an 8 mm implant.

Di osso verticale

Yes, but the available bone height is between 7 and 8 mm.

Eh ma stiamo tra i 7 e gli 8 mm

Again, the planned crown position should guide the choice among the various options: vertical augmentation or GBR, or simply soft-tissue augmentation.

I would also place a shorter implant and allow the prosthetically driven position to guide treatment. I would perform slight expansion and compaction with Black Ruby tips and manage the buccal aspect using a connective-tissue graft or a dermal matrix, which is my latest approach. In short, I would keep the procedure as simple as possible because the baseline bone volume appears adequate.
