Immediate implant in an upper first molar: interradicular septum with Elevate

But if we do drilling with normal drill this delicate bone will get lost during drilling
I used hydraulic kit of magentic mallet to prepare delicate interseptal bone
Ideal position for implant is interseptal bone
As remaining bone below sinus was 3-4 mm there is chance of implant sleeping into sinus during healing so to prevent it i used multi cover screw
To place implant in such delicate osteotomy is also challenge there is chance of slippage of implant in sinus , so we need active implant I use Gdiff 5x7
As osteotomes of magnetic mallet push the bone side wise and apical also we got expansion as well as sinus lift
Natural oro antral communication present on extraction of palatal root
So 2 options - 1 ) densah bur 2) magnetic mallet
I used octacalcium phosphate as a bone graft
Well done and the case is pretty well . But to be clear plz write down the tools of MM used with the case and why chose 5/7 mm not 5/10 specially the Magnatic will not removing the bone around but condensing ؟
And finally , y mentioned u applied xenograft but plz what mount of bone y used I mean 1 gram or more Cheers
Elevate kit was used - its special kit for sinus lift
5x7 because remaining bone was 3.5-4 mm so not advisable to lift more then 3-4 mm otherwise chances of membrane rupture ans as I mentioned in palatal root there was natural oro antral communication so if we lift too much chances of that hole in increase in size
No i have applied octa calcium phosphate which is not xeno but synthetic
Ok , I see but why you applying it instead of xeno؟
I see .. cheers dr
Despite what y used it a good choice in your case Any way , the case is fabulous Cheers
Xeno has very low tunover in sinus as it is closed cavity i need something which get changed in vital bone
Thank you for appreciation
Cheers man I have already got it and Thanks for the clarity
