Symptomatic molar: distal root fracture or inadequate treatment of the mesial root canal

13 messages · In conversation order

I would appreciate your diagnostic opinion. Could there be a horizontal fracture of the distal root? I cannot detect any clinical evidence. What do you think? The patient is symptomatic. Initially, I attributed the symptoms to the inadequately treated mesial root canal.

Consiglio diagnostico se possibile Radice distale... Frattura orizzontale? In bocca non ne ho evidenza A vostri avviso? Pz sintomatico Io inizialmente davo la responsabilità alla radice mesiale trattata in modo incongruo

Thank you.

The image is difficult to interpret. If no fracture is present, there nevertheless appears to be a defect near the furcation, where the previous treatment nearly caused a perforation. The mesial root filling is short and should be reassessed. Check static and dynamic occlusion carefully: ask the patient to chew and assess protrusive and lateral excursions. Judging by the pulpal calcification of the second molar, the patient is either very elderly or almost certainly clenches.

Si vede poco e male. Se non c' è frattura sicuramente sembra di vedere qualcosa a livello della formazione dove hanno quasi perforato. La cura mesiale è corta e da rivedere. Controlla bene occlusione in statica e dinamica, ovvero fai masticare paziente. E fai fare protrusive in avanzamento laterale. O è molto anziano o serra di sicuro a guardare la calcificazione pulpare del settimo.
