I evaluated the quality of the septum that was not so mineralized. Due to this bone consistency I directly used the 200 tip til the sinus floor. After that I used the 300 point causing the fracture of the floor. I started putting a little square of collagen sponge (just to protect the membrane) and subsequently the granules of bone graft. Finally I inserted the fixture.

With such a diameter, I imagine you use a drill in the first section (given the difference in diameter even with the largest MM tip). Generally, if you use a drill, to what depth?

Actually, the consistency was such that I did not need a drill. Once the central hole had been made, I used the flat head of the 300 instrument around the edge of the circumference to widen it slightly, also bringing up a little bone. The preparation was done by eye so that I could enter with the implant apex (which has a diameter slightly wider than the 300); then screwing in the implant condensed the bone laterally and provided stability.
In realtà la consistenza era tale che la fresa non mi è servita. Una volta praticato il foro centrale, ho utilizzato la testa piatta dello strumento 300 sul bordo della circonferenza al fine di allargarlo leggermente tirando su anche un pochino di osso. La preparazione è stata ad occhio, al fine di riuscire ad entrare con l’apice dell’impianto (che ha un diametro poco di largo del 300); poi l’avvitamento dell’impianto ha condensato l’osso lateralmente e dato stabilità.
If, instead, you had found good density in the “entry” cortical bone, would you only have created the entry with the last suitable drill for that implant?
